Depending on how you use Toolbox Genomics, registration information may include:
We collect information from a questionnaire at the time of account registration. We collect information such as self reported diagnoses, family health history, nutrition, and diet, to help us create a more personalized report. You may opt out of answering this questionnaire.
Once you use the cheek swab to collect your sample and mail it to the lab, your DNA will be sequenced. We use your genetic data results to generate your reports. We remove all personal information (ie: name, birthdate, etc. ) and store your raw genetic information, so it may be used to create additional reports for you upon request. The genetic sample submitted is destroyed after sequencing is completed. At any time, you can request your raw genetic data be destroyed by emailing: firstname.lastname@example.org. Please note, if you decide to delete your raw genetic data, you will have to re-sample if you would like to obtain new reports.
